Dear Leketia,

Your request is not complete as your statement/documentation does not meet our requirements. Please review the information below and provide adequate documentation. 

Documentation may include, but is not limited to:

  • Note from a doctor identifying the dates of medical incapacitation
  • Flight itinerary or receipt displaying the dates of travel for emergency
  • Receipt for towing services or car repairs displaying the date of service
  • Detailed candidate statement which clearly defines the circumstances including the reason why formal documentation was not obtained

I hope this message finds you well. I am writing to request a rescheduling of my exam that I unfortunately missed due to a family emergency. My mother-in-law was recently admitted to the hospital due to a serious medical issue, and I was unable to attend the exam. I apologize for any inconvenience this may have caused and kindly ask if it would be possible to waive any associated fees with the missed exam.
