Your request is not complete as your statement/documentation does not meet our requirements. Please review the information below and provide adequate documentation. 

Examples of Accepted Documentation 

  • Recommendations of qualified professionals
  • Proof of past testing accommodations
  • Documented observations by educators
  • Results of psycho-educational or other professional evaluations
  • An applicant’s statement of his or her history regarding testing accommodations

Please note, the above documentation does not have to disclose a disability, it can be a simple note from a doctor recommending a reasonable accommodation.

Please resubmit the accommodation request with the above requested documents. I also highly recommend cancelling your upcoming scheduled exam as a request could take up to 45 business days to be approved and finalized.
