ACCOUNT LOCKED & UNABLE TO SETUP TWO FACTOR AUTHENTICATION:
I have reviewed your previous instructions and I have reset my password multiple times. Even after my account was reset the blue hyperlink to "set-up two factor authentication" does not do anything. There is no option to provide a code or set up for two-factor log in. I have checked my security settings and enabled pop-ups, redirects, location, etc. I have also tried logging in from my phone and the link does not work either.
