I have ADAD and am requesting more time if needed

when I signed up for my exam my last name was Murdock, April 3 i got married and my last name is now Vculek. My driver's license says Vculek now as well. What do i need to do?

Request for time in a half for the HUD certificate. What information do I need to submit ?

The confirmation email you received when you registered for the Certification Exam included the amount you paid for the exam and serves as a purchase receipt.

You may also generate a receipt by logging into HUDHousingCounselors.hud.gov, visiting the Exam Status Center via the Exam Pre-launch page and clicking "Receipts" in the blue header. The receipt will not stipulate payment type.

Reason for Accommodation: Request for HUD exam. I have failed the exam several times, and was advised to request an reasonable accommodation. I had a severe head injury several years ago, from a car accident with a tractor trailer. I have memory issues, and suffer from anxiety when I take this test. I cannot read thru the questions that quickly, nor understand them. I am in need for more time, and a pencil with paper, and water. I can use the HUD calculator.
