Reason for the Accommodation Request: I have taken the exam twice already despite my Testing Anxiety, no test by a medical professional have been done; yet, it's something that I have struggled with my whole life and I am fully aware of myself in these situations.

Type of Accommodation Needed: Private Room for Testing purposes plus water and snacks
