- Amount paid
- Date of Payment
- Last four digits of credit card
- Name on credit card if different from your own
- Credit card type (Visa, Master Card, American Express etc....)

I have attempted to sign in and schedule the test online several times and the error 3DSecure Failure message: "Not Authenticated/Account Not Verified; Transaction denied" keeps occuring. I have contacted my bank and they have suggested that I contact your support line as the funds are available and no issue has been identified on their end. Is it possible to perform the transaction over the phone or some other platform?
