• Three Kryterion Test Center locations in order of preference
• Three preferred dates (at least 45 days from the date of your request submission)
• Three test start times which apply to ALL submitted dates 
• The total number of test takers expected to register
• Please also include any questions or comments in your response or call Support at 210-489-1461 if you need immediate assistance.

If Support does not receive a formal request including, the information above, by [DATE] we will assume the matter is resolved and close this ticket.
