Where am I able to get the QR code for the two-step authentication, so that I can set it up on my mobile device.

I have been locked out of my account but when I use the password received in the email from forgot password. The system does not allow me to sign in again using The New Password. How will I ever be able to change my actual password, so I don't have to click on I forgot my password. Please advise.
